The Perimenopause Diet: A Starting Point for Brain Fog, Anxiety and Beyond

You've snapped at someone you love over something small, then spent the rest of the day feeling terrible about it. You've walked into a room and forgotten why you went in. Or maybe your symptoms look a little different, the awkward hot flush in a meeting, or waking every night at 3am for no reason at all. Perimenopause touches more than most people talk about.


Anxiety and brain fog are two of the things I hear about most often in clinic, usually from women who've quietly decided they'll just have to live with it. You don't. Other typical peri symptoms include lacking in energy and motivation, poor sleep, reduced libido, changes to skin and hair, vaginal dryness and low mood.


For more than 14 years I've worked with women navigating perimenopause and menopause, and I never stop being amazed by how often they're told it's just something they have to put up with.


Once you understand what's shifting hormonally, you can get a better understanding of tools that can help to manage this.  These can be nutrition, lifestyle or medication related. 


What is Perimenopause?

Perimenopause is the phase leading up to menopause, where your body gradually produces less oestrogen. This stage can last anywhere from a few months to several years, typically beginning in your 40's. Hormonal shifts and fluctuations during this time can be severe. Typically our progesterone drops first, which can be destabilising for your brain and can create real imbalances in neurotransmitter levels, leading to puberty-like mood swings, anxiety, aggression (road rage anyone?) and depression.

 

Oestrogen can go from high to low seemingly randomly, and this is what is responsible for the classic symptoms like longer and heavier cycles, night sweats and brain fog, but also many lesser-known symptoms, like an increase in migraines, painful joints and itchy skin.

 

So, whether you opt for HRT or not, the body still needs the right nutrients and care through this transition.

 

The Basics of Nutrition in Perimenopause

None of this is about willpower, and it's not about one single food, food group or supplement fixing everything either. We'll start with the nutrients that support your brain and mood, because that's where the impact tends to be felt first and hardest, then work through the ones that matter for your energy, hormones and long-term health.

 

Oestrogen influences the brain in multiple ways. It affects neurotransmitter systems, how the brain responds to stress, communication between brain cells and, for many women, how sharp their thinking feels from day to day.

 

B vitamins, magnesium and omega-3 fats all play important roles in supporting normal brain and nervous system function.

 

Protein, phytoestrogens, calcium and vitamin D matter too, supporting muscle, bone, healthy ageing and, in some women, helping with symptoms such as hot flushes. But the brain-supporting nutrients are worth understanding first, because they're doing the most immediate work.


Does Perimenopause Cause Brain Fog, and What Helps?

Brain fog in perimenopause is real, it isn't a lapse in effort or a sign you're losing your edge. As oestrogen fluctuates, so does its effect on the neurotransmitters and neural connections that keep your thinking sharp, and B vitamins help support many of the same brain processes. They won't switch brain fog off completely, and they're certainly not a cure. But because they're essential for normal brain function, making sure you're getting enough is one sensible place to start.


Cognitive Function

Hormonal fluctuations, particularly the decline in oestrogen levels, can contribute to changes in your brain such as memory lapses and difficulty concentrating. One thing I find frustrating is how often these symptoms are dismissed as simply being "busy and stressed" or "getting older." Brain imaging studies have shown temporary changes in brain structure and activity during the menopausal transition, much of which appears to adapt over time. Researchers including Professor Lisa Mosconi have helped bring greater attention to this area.

 

Oestrogen influences neurotransmitter function and neural connectivity directly, and B6, B9 (folate) and B12 all play a role in that same system. B6 helps produce neurotransmitters including GABA, serotonin and dopamine, both involved in mood stability and cognitive function. B12 maintains myelin, the protective sheath around nerve fibres, keeping nerve signals moving efficiently. Folate is important for DNA synthesis and repair, and works alongside B12 to protect brain health.

 

It's also worth remembering that brain fog isn't caused by falling oestrogen alone. Poor sleep, stress, anxiety, hot flushes, low mood, iron deficiency, thyroid problems and simply trying to juggle too much can all contribute. Good nutrition won't solve every cause, but it provides the building blocks your brain needs to function as well as it can during this transition.

 

Cortisol Regulation

Stress activates your body's stress response system, increasing the release of cortisol. While cortisol is an important hormone that helps us respond to challenges, chronically elevated levels can affect sleep, energy, concentration and mood, making many of the symptoms of perimenopause feel even more pronounced.

 

B vitamins play an important role in energy metabolism and nervous system function, and B5 in particular supports your adrenal glands in producing cortisol. B vitamins won't prevent the effects of chronic stress, but correcting a deficiency helps make sure this system is working as it should


Energy Production

B vitamins also help release energy from your food. B1, B2, B3, B5 and B6 are all involved in turning carbohydrates, fats and protein into fuel, and B12 and folate are needed to make the red blood cells that carry oxygen around the body. When either is running low, tiredness is often the first thing you notice.

Supplements

B-complex supplements can provide a balanced dose of B vitamins, especially beneficial for those with dietary restrictions or absorption issues.

Can Magnesium Help With Perimenopause Anxiety?

Magnesium has a direct, calming effect on the nervous system, which is why it's one of the first things I look at with clients dealing with anxiety, racing thoughts or a mind that won't switch off at night. It works by regulating GABA, the neurotransmitter responsible for relaxation, so when magnesium is low, that calming signal doesn't fire the way it should. It also helps relax muscles, which is useful if cramps, restless legs or general tension are part of the picture too.

 

Magnesium-Rich Foods

One of the easiest ways to increase your magnesium intake is through food. Some excellent dietary sources include:

  • Leafy Greens: Spinach, kale and bok choy

  • Nuts and Seeds: Almonds, pumpkin seeds, and sunflower seeds

  • Whole Grains: Quinoa, brown rice, and oats

  • Legumes: Lentils, chickpeas, and black beans

  • Dark Chocolate: Choose chocolate with at least 70% cocoa for a magnesium boost.

 

Tip: Try almonds and dark chocolate for a magnesium rich snack. You can also add leafy greens to your meals or incorporate quinoa and beans into salads or soups for an added magnesium punch.

 

Magnesium Supplements

There are various forms of magnesium supplements available, each with slightly different benefits:

  • Magnesium Glycinate: calming, highly absorbable and gentle on the stomach, ideal for stress and sleep.

  • Magnesium Citrate: easily absorbed, can have a mild laxative effect, useful for constipation.

  • Magnesium Threonate: often recommended for its ability to cross the blood-brain barrier, making it particularly effective for cognitive function, mental clarity and stress relief.

  • Magnesium Malate: often used for muscle relaxation and energy production, a good option for muscle tension or cramps.

 

 

Recommended Dosage

The Adequate Intake for magnesium sits at 300mg a day for women, per EFSA, the reference used across Ireland and the EU, though perimenopausal women dealing with more stress or disrupted sleep sometimes do better on somewhat more. Worth a chat with a healthcare professional to land on the right amount for you, and to rule out any interaction with medication you're on.

 

I had a client whose anxiety and sleep improved noticeably once we corrected which form of magnesium she was on. She'd been taking one that wasn't well suited to her, and at a dose too low to do much. It's a mistake I see often, the right idea, the wrong form, or not enough of it to make a difference. Worth flagging if you're prone to IBS or a sensitive gut, magnesium citrate in particular can make diarrhoea worse, magnesium glycinate is usually gentler.

Can Omega-3s Ease Perimenopause Inflammation, Joint Pain and Brain Fog?

Inflammation isn't the enemy people assume it is. It's your body's repair system, doing exactly what it should when you're fighting an infection or healing an injury.

 

The challenge during perimenopause isn't inflammation itself, but that declining and fluctuating oestrogen can shift the body's inflammatory balance. This may contribute to some of the symptoms women experience, including joint aches, changes in mood and, potentially, brain fog.

 

Omega-3s, particularly EPA and DHA, help keep that inflammatory response in balance rather than switching it off completely. They're also a key building block of cells throughout the body, especially in the brain and nervous system, which is why they matter for more than inflammation, they support brain function and overall health through the menopausal transition.


Cognitive Function

DHA is a major structural component of the brain and is particularly concentrated in the membranes of brain cells. It helps those cells communicate efficiently, supporting memory, learning and concentration. As hormone levels fluctuate during perimenopause, many women notice that they feel less mentally sharp or struggle to find words they once reached for easily. While omega-3s aren't a magic solution to brain fog, ensuring an adequate intake provides your brain with one of the key building blocks it needs to function at its best.

 

Mood and Stress Response

The emotional rollercoaster of perimenopause isn't "all in your head." Changing hormone levels affect the brain chemicals involved in mood, stress and emotional regulation, while ongoing stress can amplify many of the symptoms. Omega-3s appear to support healthy brain signalling and may help improve mood and resilience to stress. Some research also suggests they can help dampen the body's stress response, making it easier to cope with the demands of everyday life. They're unlikely to eliminate anxiety on their own, but they can be an important part of the bigger picture.

 

Joint Health

If you've found yourself waking up feeling stiff, sore or wondering why your knees, hips or hands suddenly ache more than they used to, you're certainly not imagining it. Many women notice more joint discomfort during perimenopause, partly because the anti-inflammatory effects of oestrogen begin to decline. Omega-3s help regulate inflammation throughout the body, and studies suggest they may help reduce joint pain and stiffness, supporting better mobility and making it easier to stay active.

 

Sources

Food

Oily fish such as salmon, mackerel, sardines, trout and anchovies are the richest dietary sources of EPA and DHA. Plant foods including flaxseeds, chia seeds and walnuts provide ALA (alpha-linolenic acid), a plant-based omega-3 fat that the body can convert into EPA and DHA, although only in small amounts.

 

Supplements

Fish oil and algae oil provide EPA and DHA directly and may be worth considering if you don't eat oily fish regularly. Algae oil is an excellent option if you follow a vegetarian or vegan diet. As with any supplement, if you're taking blood-thinning medication or have a bleeding disorder, check with your GP, pharmacist or healthcare professional before starting an omega-3 supplement.

Why Does Perimenopause Increase Your Protein Needs?

Perimenopause quietly changes how much protein your body needs, and most diet advice hasn't caught up. As oestrogen declines, women naturally begin to lose muscle mass and strength unless they actively work to preserve it. At the same time, changes in body composition can make it easier to gain body fat and harder to maintain the muscle that's so important for metabolism, healthy ageing and long-term health.

 

The women I see in clinic are rarely under-eating. They're often eating quite carefully and trying to make healthy choices. What's usually missing is protein, and not by a small margin. Current evidence suggests that many women in midlife may benefit from around 1.2–1.6g of protein per kilogram of body weight each day, ideally spread across meals rather than eaten mostly at dinner. For most women, that means aiming for 20–30g of protein at each meal - roughly a palm-sized portion of chicken or fish, 200g of Greek yogurt, three eggs, or a generous serving of tofu, beans or lentils alongside other protein-rich foods.

 

Protein does far more than help build muscle. During perimenopause it helps preserve the lean muscle mass that declining oestrogen is working against, supports bone health, contributes to tissue repair and immune function, and helps keep blood sugar and appetite steadier throughout the day. That can translate into better energy, fewer cravings and feeling fuller for longer.

 

Protein and strength training also work hand in hand. Eating enough protein provides your body with the building blocks it needs, but regular resistance exercise gives your muscles the signal to maintain and rebuild themselves. Together, they're one of the most powerful combinations we have for supporting healthy ageing.

 

Simple ways to increase your protein intake

  • Choose eggs or Greek yogurt at breakfast instead of relying on toast or cereal alone.

  • Add beans, lentils, tofu or extra chicken to soups, curries, pasta dishes and salads.

  • Pair fruit with a protein source such as Greek yogurt, cottage cheese or a handful of nuts for a more satisfying snack.

  • Aim to include a good source of protein every time you eat, rather than saving most of it for your evening meal.


What Are Phytoestrogens, and Do They Help with Perimenopause Symptoms?

A cup of soy milk, a tablespoon of ground flaxseed and a small handful of edamame. It doesn't sound like much, but for some women, regularly eating these foods can take the edge off hot flushes and other menopausal symptoms.

 

Phytoestrogens are naturally occurring compounds found in certain plant foods. Their structure is similar to oestrogen, allowing them to bind to oestrogen receptors in the body and produce much weaker oestrogen-like effects. They won't replace the hormones your ovaries are producing less of, but for some women they can help soften the impact of fluctuating hormone levels. Like most things in nutrition, consistency matters more than perfection.

 

Researchers believe phytoestrogens work in several ways. As well as interacting with oestrogen receptors, they may influence inflammation, antioxidant activity and even the gut microbiome. While we're still learning exactly how they work, the evidence is strongest for helping to reduce the frequency and severity of hot flushes in some women.

 

Good food sources of phytoestrogens

  • Flaxseeds – one of the richest sources of lignans. Ground flaxseed is easy to stir through porridge, yogurt or smoothies.

  • Soy foods – tofu, tempeh, edamame and soy milk contain isoflavones, the type of phytoestrogen with the strongest evidence for easing menopausal symptoms.

  • Legumes – lentils, chickpeas and beans provide smaller amounts of phytoestrogens, along with fibre and plant protein.

  • Sesame seeds – another useful source of lignans that can easily be sprinkled over salads, vegetables or breakfast.

 

Many of the studies showing benefits have used around 40-80mg of isoflavones a day, an amount that can often be achieved through food rather than supplements.

 

A simple day's intake might include a glass of soy milk, 100g of tofu or tempeh, a tablespoon or two of ground flaxseed and a serving of chickpeas or lentils. You don't need to eat all of these every day, but including them regularly is likely to be more beneficial than having them only occasionally.

 

One concern I hear surprisingly often is whether soy is safe. The good news is that the current evidence is reassuring. For most women, eating whole soy foods as part of a balanced diet is considered safe and may even provide wider health benefits beyond menopause.

 

Supplements containing soy isoflavones or red clover are also available and may help some women. Herbal products such as sage are commonly marketed for menopausal symptoms too, although they work differently and the evidence is more variable. If you're considering supplements, especially if you're taking medication, HRT or have a history of hormone-sensitive cancers, it's always worth discussing them with your GP or healthcare professional first.

 

Think of phytoestrogens as one piece of the puzzle rather than the whole answer. They may help reduce symptoms such as hot flushes for some women, while the nutrients we've already talked about support your brain, muscles, bones and nervous system. It's the overall pattern of your diet that has the greatest impact on how you feel during perimenopause.

Why Does Perimenopause Increase Your Risk of Osteoporosis?

This is the one nutrient pairing in this article that isn't mainly about how you feel today. Oestrogen plays a vital role in maintaining bone strength, and as levels decline during perimenopause, bone is broken down faster than it's rebuilt. The result is a gradual loss of bone density that happens silently, often without any symptoms.

 

That's why looking after your bones now is an investment in your future health. Building and maintaining strong bones during midlife can help reduce your risk of osteoporosis and fractures in the years ahead.

 

Calcium is the main mineral that gives bones their strength, while vitamin D helps your body absorb and use that calcium effectively. One doesn't work well without the other, so it's important to pay attention to both.

 

Good sources of calcium

  • Dairy foods: Greek yogurt, milk and hard cheeses provide calcium along with high-quality protein.

  • Fortified plant milks: Soy, oat and almond milks fortified with calcium are excellent dairy-free alternatives.

  • Leafy green vegetables: Kale, pak choi and spring greens are among the best plant sources.

  • Legumes: White beans, chickpeas and other beans provide calcium, fibre and plant protein.

 

Good sources of vitamin D

  • Oily fish: Salmon, mackerel and sardines are among the richest natural sources and also provide heart-healthy omega-3 fats.

  • Egg yolks: A modest source of vitamin D, alongside protein and healthy fats.

  • UV-exposed mushrooms: Certain varieties provide useful amounts of vitamin D, making them a good option for those following a plant-based diet.

  • Fortified foods: Some breakfast cereals and plant milks are fortified with vitamin D.

 

Unlike calcium, vitamin D is difficult to obtain from food alone. Most of our vitamin D is produced when sunlight reaches the skin, but in Ireland and the UK, this is only possible for part of the year. For this reason, many people benefit from a vitamin D supplement, particularly during the autumn and winter months, although individual needs vary.

 

Nutrition is only one part of protecting your bones. Regular weight-bearing activities such as walking, dancing or climbing stairs, together with resistance exercise like strength training, encourage your bones to stay strong. Combined with enough protein, calcium and vitamin D, they're one of the best investments you can make in your long-term health.

 

Unlike hot flushes or brain fog, weakening bones don't usually let you know they're happening. That's why prevention matters so much—by the time osteoporosis is diagnosed, you've often already lost a significant amount of bone strength.

Perimenopause looks different for every woman, brain fog for one, anxiety for another, hot flushes or joint pain or none of the above. What's consistent is that something hormonal is genuinely shifting, and food is one of the few tools you have real, daily control over while it does.

 

None of this replaces proper care if you need it, and if what you're feeling is more than nutrition can reasonably touch, that's worth a conversation with a professional, myself included. But understanding what's happening, and having a starting toolkit rather than a vague sense that you should eat better, is worth something on its own.
